DAY BEFORE PROCEDURE

• _____ Follow instructions given to you by your physician, ask your physician if you should take your medication on the day fo the procedure.

• _____ Avoid alcoholic beverages.

• _____ You MUST have transportation to and from the Endoscopy department and someone with you at home.

BRING:

• _____ Insurance information or cards.

• _____ List of medicines you take

• _____ List of allergies

DIRECTIONS (See map below)

• From Ft. Harrison, turn west on Pinellas Street.
• At 4-way stop sign you will see public parking lots
(a golf cart will provide transportation to Adler entrance as needed)
• Enter Adler building at the end of Pinellas Street. (large blue canopy)
• Take the 1st set of elevators on left to the 2nd floor
• Follow sings to "Endoscopy"

*NOTE: If you need wheelchair assistance, use phone at Adler entrance, ext. 27296

***You may be instructed to arrive earlier if other tests are to be done.
